You may notify the Records Office of a change of name, address, or phone by printing, completing and submitting the Notice of Change of Name, Address, or Phone Number form available below. This form is available in two formats -- Microsoft Word and Adobe PDF.

These forms cannot be accepted without an appropriate legal signature. For this reason, neither fax nor online submission of this information is allowed.

Instructions

To use the Adobe PDF form:
