What We Do

CPS provides a broad range of case management services including post-hospitalization treatment planning, resource bridging, skill building and academic coaching for students on the Autism Spectrum.

PROGRESS MONITORING

  • Develop and implement treatment plans based on needs
  • Guide in the development of skills or strategies for coping with academic and/or behavioral health issues
  • Modify treatment activities or approaches as needed to comply with any changes

REFERRAL / BRIDGING SERVICES

  • Refer to community resources or to specialists as necessary
  • Follow up sessions to ensure referrals and/or information provided met needs.

BEHAVIORAL HEALTH FOLLOW-UP

  • Provide follow-up after inpatient/residential treatment
  • Coordinate required services or to resolve emergency problems in crisis situations
  • Serve as advocates to the University, significant others and/or the community

CRISIS CASE MANAGEMENT

  • Assess for risk of suicide attempts and/or ideations
  • Assess capacity to cope during and/or following a crisis
  • Prioritization of needs and development of action plan
  • Coordination of care with both formal and informal supports
